Left side rider footpeg

I went for a ride today and noticed that my left side peg was rotating around. It was not coming off, but can now spin freely. I cannot budge the bolt either way to get it off to see what has happened.

Can someone explain how the peg assembly goes together and keeps the peg from spinning?
It is obvious that someone has replaced it but they must have left out a washer, or key that keeps it still and cranked it down extra tight.

This is an 02 model.

Different footpeg styles

So I just bought a left side riders footpeg assembly that was from an 04.

It is different than my current one. This one has a no bolt through to hold the footpeg, the footpeg holder is welded on.

My question is, what years have the bolt on peg holder and what years have the holder casted as part of the bracket?
