has anyone ever had one of these come loose or off while riding,forcing sealing ring(white gasket to come out)

lucky me I was riding a buddies '74 and you guessed it. the sleeve came up and gasket spit out. oil , oil, oil

well i feel bad and now will fix it but didn't know if there where any tips or tricks to fix without taking head off and to keep this from happening again.(this a first for me seeing one pop up)

well the only thing i can come up with is maybe the o ring on the exhaust tappet gave out and oil pressure blew out the push rod gasket.

what a pain is the butt to fix. 30cent part/ 3-4 hours of my time:
