It is certainly possible that a bad cap was an issue the first time. I haven't seen much posted here about that being a common problem and mine is still good at 40k miles.
As to your issue after the ride home, either they damaged the cap's gasket, or more likely left something loose like the bleed screw. It is also very easy to damage the plastic elbow on the overflow tube when removing/replacing the tank.
