Loose mirrors

I had the same problem and the same fear of stripping the bolt. The mirror stalk appeared to be bottoming against the tightening bolt.

I pulled the mirror shaft completely free of the lever/switch pod and ground/filed a slight amount off the bottom of the shaft to prevent it from bottoming out against the bolt. Hasn't slipped since!
